Overview

Join our dedicated team as a Departmental Secretary (Outcomes Management) and play a vital role in providing essential administrative support to our department. Your efforts will ensure smooth coordination and transition of patients to other facilities or agencies, under the guidance of discharge planners. This role requires excellent communication skills, attention to detail, and proficiency in medical terminology.

Responsibilities

  • Administrative Support: Provide comprehensive administrative support to the outcomes management department.

  • Agency and Facility Coordination: Initiate contact with agencies (e.g., Home Health, DME, Dialysis) and other facilities (e.g., Nursing Homes, Rehab Hospitals) regarding resources and admission availability via ProviderLink phone.

  • Patient Transition: Coordinate and complete required paperwork and phone calls to transition patients to other facilities or agencies, under the direction of a discharge planner.

  • Documentation Issuance: Issue necessary documents to patients and their families as directed by the case manager.

  • Communication: Serve as a liaison between patients, families, healthcare providers, and external agencies to ensure smooth transitions and effective communication.

Qualifications

  • Education: High school graduate or equivalent required.

  • Work Experience: Two to three years of experience in a medical office/clinic or hospital unit setting preferred.

  • Skills:

    • Strong personal computer skills, including proficiency with Microsoft Office programs.

    • Knowledge of medical terminology preferred.

    • Excellent verbal and written communication skills.

    • Strong organizational skills and attention to detail.

    • Ability to manage multiple tasks efficiently.
